Home Health Agency Participant Registration
There are three steps to becoming a Home Health Quality Improvement Participating Agency:
- Discuss the benefits of participation in this national campaign with appropriate agency managers and staff. Refer to the Recruitment Package for information on benefits of participation.
- Join the campaign by completing the online provider registration form. You will need your six digit Medicare Provider Number.
- Your agency will receive an electronic welcome package within two to three weeks of registration. This will include an informational letter, the exclusive HHQI logo and a certificate of participation.
