This agency has identified phone monitoring and frontloading visits as a major focus for their agency.

All the information is helpful. We would like for all the staff to review it, but time/scheduling is a problem. That is one reason the Fast Track information is usually used the most along with selected tools that may work with what has already been started on previous POAs.

It is my opinion that agencies use the information provided and individualize it. I do find the packages somewhat overwhelming. Sometimes it is more information then we can assimilate. I pick & choose from the packages what I believe is most important. Nurses will not learn if too much is given at one time.

Again, great comment. Individualize, individualize, and individualize these packages. Pick and choose to meet your agency and staff needs.

I think it is GREAT to have these tools already prepared & ready to be implemented. These tools have saved me so much time in research and preparation! Keep them coming!!!

It would be nice to have the tools in Microsoft Word format rather than PDF as we don't have software for conversion (to modify).

Some of the tools are available in MS Word. Go to the package Web page, scroll down and see several tools available as individual documents. Some of these are in Word. Unfortunately we can’t make all of them available in Word. One tool that we have received frequent requests for in the MS Word version is the ‘My Emergency Plan’. This is already available in Word on the Emergency Care Planning Package Web page.

The packages are GREAT! It is amazing how much information is available. We try to bring in something from every package. We have made CD's from every package and made them available to all clinicians and aides. They are allowed to "check them out" overnight or come in and listen to them in house.

I really like the "Physician Connection, " We have some clinicians who are timid about asking for exactly what they know they need. I'm hoping these physician quotes will help.

We have a physician workgroup that meets monthly to discuss the home health campaign and offer insight into working with physicians. Some of their comments and input is used for the Physician and Hospital Connection page(s).

I love the useful information and ideas for implementation. Our staff gives back positive feedback on the clinical guides.
